Girls' basketball didn't join the YAIAA until 1975-76, when 15 teams formed the inaugural season. Dallastown dominated the early years, going undefeated in its first three seasons and capturing three league titles in a row over a field that included Central York, Dover, Hanover, Kennard-Dale, Northeastern, Red Lion, Spring Grove, South Western, Susquehannock, Vo-Tech, West York, William Penn, York Catholic and York Suburban.

The league grew to 16 with the addition of Eastern York in 1976-77 and lost one when Vo-Tech dropped its program from 1987-88 to 1992-93. Delone Catholic, Littlestown and New Oxford joined in 1992, and the addition of Bermudian Springs, Biglerville and Fairfield in 2004-05 brought the league to its current formation.
